What lands on a homebound neighbor's plate affects blood pressure, blood sugar, healing, and how often they end up in an emergency room.

Our approach

Nutrition, not just calories

Our commercial kitchen prepares balanced meals with attention to sodium, portion size, and protein — the levers that matter most for the seniors and chronically ill neighbors we serve. Fresh produce moves through our food bank alongside shelf-stable staples so pantry partners can offer more than cans.

Weekly delivery gives us something a prescription cannot: eyes in the home. Visitors notice when someone has stopped eating, lost weight, run out of medication, or seems confused, and they report it to program staff.

Hospitals, health plans, and care managers

We work with discharge planners, care managers, and community health teams who need a reliable food and companionship pathway for patients going home alone. Referrals are simple, there is no cost to the patient, and our visitors provide a recurring touchpoint between clinical appointments.
